Shown before the first health-related question in the Amaina web onboarding questionnaire at amaina.health ("the questionnaire" / "the quiz"), and linked from the quiz consent step. It adds to — and does not replace — our main Privacy Policy, Cookie Policy, and Medical Disclaimer. Where this notice and the main Privacy Policy differ about the questionnaire, this notice controls.
1.Why we ask these questions
Amaina is a migraine tracker & doctor-visit-prep product. To tailor your experience — which features we surface first, and a personalized summary — the questionnaire asks a small number of health-related questions. You don't have to answer them to browse amaina.health or to get the app; the questionnaire is there to make Amaina more relevant to your situation.
2.What we ask (and what counts as sensitive)
The current questionnaire asks:
- How long you've been dealing with migraines (duration)
- How many migraine attacks you have per month (frequency)
- Whether you experience aura symptoms (e.g. visual disturbances, sensory changes, speech/language, none, or other)
- What matters most to you right now (your goal — e.g. understanding causes, tracking for a doctor visit, spotting patterns, keeping track of medications)
At the end, only if you choose to save your summary, we ask for your name and email.
Under privacy laws, your migraine-related answers are "consumer health data" (Washington My Health My Data Act and similar US state laws), "sensitive personal information" (California CPRA), and "special category data" (EU/UK GDPR). We collect and handle them under the heightened protections those laws require.
3.Your consent (two separate steps)
- Health answers — opt-in, up front. Before the first health question, you actively agree via an unchecked box ("I agree to Amaina collecting the health-related answers I give in this questionnaire, as described in this notice"). This is your opt-in consent to collect this information (Washington My Health My Data Act and similar) and, for EU/UK visitors, your explicit consent under GDPR Article 9(2)(a). The box is never pre-ticked.
- Name & email — separate step. At the summary step, a separate, unchecked box authorizes us to process your name and email as described in the Privacy Policy. Providing them is optional.
- Sharing is separate. We do not sell or share your health answers, and we would not do so without asking you separately, in writing, first.
- Withdraw any time by stopping the questionnaire or emailing legal@smart-it.io. Withdrawing doesn't affect anything we lawfully did before you withdrew.
4.How we use your answers
- Personalize your experience — which features and content Amaina surfaces first.
- Build your personalized summary — the recap you view in the Amaina app.
- Improve the questionnaire and product — in aggregate.
We do not: use your answers (or health inferences from them) for advertising / ad targeting; send them to Meta, Google, or any advertising platform (our web pixels are used only for aggregate ad-performance measurement and never receive your health answers); build custom, lookalike, or health-interest audiences; or make any automated decision with a legal or similarly significant effect on you.
5.Whether your answers are linked to you
If you give us your email (to save your summary / create your account), we associate your answers with that email so we can personalize what you see — this is part of what you consent to in Section 3. If you complete the questionnaire without giving your email, we keep your answers only in a de-identified form for product improvement.
6.How long we keep your answers
- Linked answers (with your email): until you ask us to delete them, or up to 12 months after you complete the questionnaire, whichever comes first.
- De-identified / aggregate data: retained for product analytics without a time limit, because it can no longer be tied to you.
Deletion requests are honored within 30 days unless a law requires us to keep the data longer.
7.Who helps us process this — and AI
A small set of service providers help us run the questionnaire (hosting and form/CRM tools — e.g. HubSpot for the email/summary flow). Each is bound by contract to use your data only as we instruct.
The web questionnaire itself does not send your answers to any third-party AI service. Your personalized summary is shown in the Amaina app; any AI features inside the app (which help organize your own logs) run only under a separate, explicit AI-processing consent in the app — see our AI Processing Disclosure. We do not use your questionnaire answers for advertising, and we do not sell them.
8.Your rights
You can always see, correct, or delete your answers, withdraw consent and stop the questionnaire, limit how we use your sensitive information (California), and opt out of any "sale" or "sharing" (we don't sell or share health answers). Email legal@smart-it.io — we respond within 30 days (45 for California). If your browser sends a Global Privacy Control (GPC) signal, we honor it as an opt-out of "sale"/"sharing." 9.This is not medical advice
Your personalized summary, and anything Amaina shows you based on your answers, is informational only — a reflection of what you told us, not a diagnosis, prediction, or treatment recommendation. Any patterns we surface are observations from your own answers. Always talk to a qualified healthcare provider. See our full Medical Disclaimer.
10.Adults only (18+)
The questionnaire is for adults 18 or older. We don't knowingly collect health information from anyone under 18.
